Liability Waiver and Assumption of Risk.
I acknowledge that participation in workouts, classes, and the use of facilities and equipment at UNITY Fitness Harbourfront involves inherent risks, including the possibility of injury, illness, or death. I understand these risks and voluntarily choose to participate. I agree to assume full responsibility for any injuries, losses, or damages that I may sustain while on the premises or participating in any activities, whether arising from my own actions, the actions of others, or the condition of the facilities and equipment.I hereby release and discharge UNITY Fitness Harbourfront, its owners, officers, directors, employees, agents, affiliates, and representatives from any claims, demands, damages, or causes of action, including those arising from negligence, that may result from my participation in any activities or use of the facilities and services. I confirm that I have read and fully understand this waiver and that I sign it voluntarily, intending to be legally bound.
